We are a consulting physician practice with expertise in medication therapy guidance and antibiotic streamlining.
Our team consists of pathologists, practitioners, and clinical pharmacists, who assess your patients’ diagnoses, allergies, and medication regimens, along with their lab results to give patient-specific, evidence-based recommendations that help prescribers choose the safest, most effective medication choices.
Working with treating providers in a fashion similar to a radiologist reviewing imaging diagnostics, we review a patient’s relevant laboratory data and provide an easy-to-follow set of recommendations for drug/dose/route/time based on the lab findings, from cultures/molecular pathology results, to metabolic profiles, lipid panels, A1c’s, and other lab tests used to guide therapeutic choices.
preventing avoidable medication-related adverse events
In addition to providing evidence-guided antibiotic streamlining, we assess for:
Drug-to-Drug Interactions
Duplicate Therapies
Drug-to-Gene Risk
Diagnostic Findings That Warrant Possible Drug or Dose Changes (e.g., poor renal function)
Outdated or Missing Labs Relevant To Medication Regimen + Disease States
Treatment Gaps Outside of Guidelines (e.g., DM >40yo no Statin prescribed)
Recurrent Infections Necessitating Programmatic, Ongoing Treatment/Assessment
interprofessional consultation
Because we are a consulting practice, your providers can coordinate your patients’ care with clinicians through interprofessional consultation (CPT 99452).
Simply inform your patient that you’re requesting a patho-pharmacology review with our group, either verbally, or by adding such language to your practice’s informed consent document.
Document consent and that you are sending the patient’s case for review/recommendations and send us your patient information via secure email or through our referral portal.
We return our review the same day, or within 24 hours of receipt. And we’ll call or email you directly, should we find a drug interaction or other finding that would warrant your immediate attention.
